Use this form every time an employee is suspected of being under the influence of alcohol

 

About this document:
Ensure that the person on duty dealing with the situation is equipped to handle it effectively and fairly by printing out or having this form on hand to be completed every time an employee is suspected of being under the influence of alcohol.
The form will act as a guide reminding the manager/supervisor on duty to be aware of the important details and to make a note of them.
The completed document is used in any enquiry that follows the incident (a disciplinary hearing or an incapacity investigation) and can also be used as evidence in a CCMA case.
The form ensures consistency in treatment of employees (this is a legislative requirement) and mitigates the risks of relying on human memory for the details.
