This letter is sent to a medical professional with specific information and questions. The information obtained through the use of this letter is a requirement of a fairness when dealing with permanent or temporary incapacity cases.

This document is required to start the incapacity procedure when an Employee has informed an Employer of a specific condition, or the Employer has observed that an Employee is displaying symptoms at work that is impacting on their ability to do their job, or an Employees frequency and/or duration of sick leave has become a concern and is impacting on the ability to fulfil their responsibilities.
