Herewith some suggested wording to inform your employer of your maternity leave:
Dear (insert your Employer or Manager’s name here)
As you are aware I am currently pregnant and my expected date of birth is (insert due date here).
I would like to commence my maternity leave on (insert date here) and I would like to return to work on (insert date here).
Please let me know what you require from my side to hand over my work before I commence maternity leave.
